Benefits Counseling/ Legal Assistance
Answers questions regarding the application process for programs such as Medicare, Medicaid, Social Security, and Supplemental Insurance over the phone, through home visits, and presentations and enrollment events in the community.
Care Coordination
Persons who are age 60 and older who have been in the hospital or experienced an illness or injury may benefit from services that will promote a safe recovery. Individuals are encouraged to participate forming their care plan.
Caregiver Support
Provides information about resources, locating services, caregiver training, peer support and counseling, respite care, and supplemental services on limited basis.
211 Texas Information & Referral
Assesses the needs of the inquirer then actively links the client to the needed service by way of referral.
Nutrition/ Meals
Nutritious meals are served onsite and/or delivered to home-bound seniors in a network of congregate sites and home-delivered meal service areas throughout the region.
Ombudsman Service
An Advocate Program for nursing facility & assisted living residents. Provides advocacy to residents of long-term care facilities based on laws, regulations, and procedures.
